Utah Birth Certificate Index, 1905-1909
"This is the permanent legal record of all live births in the state filed with the Utah Office of Vital Records and Statistics in compliance with UCA 26-2-5. The information recorded in 1905 included the newborn child's full name, sex, date of birth, place of birth, whether a single or multiple births took place, the parents' names, race (color), ages, birthplaces, occupations, and marital status, and the number of children born to the mother." Utah Death Certificate Index, 1905-1965
"A death certificate is the permanent legal record of an individual's death. Each death certificate includes the decedent's full name, date of death, county where death occurred, decedent's race and gender, place and date of birth, marital status, occupation, permanent residence, place and date of burial, time of death, chief cause and contributory factors of death, and if applicable, where illness was contracted and duration of illness.
